Calling all Youth with Chronic conditions ages 13-15!
Parents, you too! Our program is family focused and we need at least one parent/caregiver to attend and help build independence, skills, health history and learn about the transition process.
You’re in Charge was designed for youth (13 to 15 years old) and their parents to build youth’s self-management skills and confidence leading to the management of their chronic health condition and navigating the health care system. Most youth with special health care needs that are treated in a pediatric setting will eventually need to be transferred to adult-based care for ongoing treatment. The education process of transition should start early, the You’re in Charge program provides information on transition process and how to prepare, youth will learn from older youth that have gone through the transition process and who now receive care in the adult system. Parents will also have the support of their peers on what has worked for other families and learn about helping their teen take on more responsibility for their healthcare.
A 3 hour workshop is offered here at the IWK. The first half of the session will focus on learning about transition. The second half of the workshop youth and parents will break into two groups, through activities, discussions, resources and tools we explore managing medication, health history and a current summary of their health, everyone will create their own personal action plan with goal setting.
It’s free! And there will be snacks & light lunch provided, (dietary restrictions will be accommodated).
