If you have been a patient of the IWK Health Centre (the former Grace Maternity Hospital and IWK Children’s Hospital), you can receive a copy of your health record by submitting a written request to the IWK Health Centre to Release of Information Office, Health Record Department, IWK Health Centre, 5850/5980 University, Halifax, Nova Scotia, B3J 3G9
