Community Health Teams

Community Health Teams offer free wellness navigation services and group wellness programming to Nova Scotians who’d like help better understanding their own health, and how to care for it. It’s a program of IWK Health and Nova Scotia Health.

How we help

Wellness navigators can help with any aspect of your family’s health and well-being such as financial support, recreation, social support, housing, employment, school supports, mental health, etc. These are health professionals who know health care, community, and government systems, and they can help you connect you to available resources, and identify what might be getting in the way of your ideal health.

Group wellness programs provide health information, strategies to make health behaviour changes, and resources. Topic areas include parenting, nutrition and weight management, mental wellness, physical activity, and managing risk factors.             

Accessing this Clinic, Program or Service

Wellness Navigators is available, by phone, Zoom, or in person, to residents of Greater Halifax Communities. Call 902-460-4560 to book an appointment or to find out more.
